AUP Renewal Form

The AUP Renewal Form must be submitted and approved each year. The form contains all of the information about changes to your AUP.


Creating the AUP Renewal Form

Go to AUPs > ACC Documents > AUP Renewal Form

Step 1: AUP Details

Select the principal investigator, contact person for this form, and AUP.

Step 2a: Renewal Year

Specify the renewal year for the AUP. A new AUP must be submitted for the 4th year renewal.

Step 2b: Confirmation of Changes to Current AUP

For each of the AUP sections, select Yes for any changes to the AUP that have not been documented by previous amendments and explain the changes. Select No if there are no changes.

If there are changes, specify the changes in the fields provided.

Mouselines Used

The form lists all of the mouselines currently listed on the AUP and the number of active cages for each mouseline.

Mouselines that are no longer needed can be removed from the AUP. New mouselines can be added using the Add Mouseline dropdown.

Research Staff

The form lists all of the personnel currently listed on the AUP and the number of active cages for each person.

Persons that are no longer working with the lab can be removed from the AUP. If a person is no longer working with the lab but still has cages, you can specify the person to whom the cages should be transferred. New people can be added using the Add Person dropdown.

Step 2c: Annual Animal Number Reporting to Animal Care Committee

The form automatically displays the number of animals allowed and the number used for the year.

Step 2d: Welfare Update

The form lists all of the mouselines currently listed on the AUP, the last time the passport was updated, the allele status, and welfare assessment status.

If the passport has no alleles associated with it, a status of 'Need to add alleles' will be displayed. If a passport has default alleles associated with it, a status of 'Contains Default alleles' will be displayed. Alleles are not mandatory but can make colony management simpler. The alleles will be included on the cage cards to allow the researcher to easily document a mouse's genotype. Including the alleles also makes it possible to find similar mouselines in the LIMS system.

Each passport includes a welfare assessment section. The information in this section is used by the TCP staff to ensure the mice are healthy. If the welfare assessment status is 'Assessment_Not_Done_Yet', update the Welfare tab on the mouseline before submitting your renewal form.

Step 2e: PCR Cell Line Testing Results

If the AUP uses biological agents, a recent PCR cell line test result must be provided.

Step 2f: Analgesics

Specify which analgesics have been used on the mice on this AUP in the past year.

Step 2g: Controlled Substance Exemption

The form lists all of the controlled substance exemptions associated with the AUP. If there are new substances being used on the AUP, list them here.

Step 5: Additional Comments

Enter any additonal comments that you think would be helpful for the ACC.

Step 6: Save the Form

Click the Save Draft button to save the form. You can do this after each agent is added. The form will not be submitted until you click the Submit button.


Submitting the AUP Renewal Form

Click on the Submit button to submit the AUP Renewal Form for review. The contact person and ACC Co-ordinator will be notified that the form was submitted.


Reviewing the AUP Renewal Form (for TCP Staff only)

To view the AUP Renewal Form, navigate to View AUP > ACC Submissions or AUPs > Pending ACC Documents and click on the view link for the form.

Add Review Comments

Enter any comments in the Administation > Admin Comments section.


Notify Contact that Revisions are Required

When all of the comments are entered, click Notify Revisions Required in the Status field. The contact person will be notified that changes to the form are required before it can be approved.


Responding to Review Comments

To view the AUP Renewal Form, navigate to View AUP > ACC Submissions or AUPs > Pending ACC Submissions and click on the view link for the form.


Update AUP Renewal Form Based on Review Comments

Based on the comments in the Administation > Admin Comments section, update the form.


Re-submit AUP Renewal Form

When all of the revisions are entered, click the Re-Submit button. The contact person and ACC Co-ordinator will be notified that the form was re-submitted.


Approval and AUP Updates (for TCP Staff only)

Once the AUP Renewal Form has been approved, the AUP can be updated with the hazardous agents.


Approve the AUP Renewal Form

Click Approved in the Status field of the Administration section. The contact person and ACC Co-ordinator will be notified that the form was approved.


Update the AUP

Click the Update AUP button to add the agents to the AUP.


Add the AUP Renewal Form to the AUP

Navigate to the AUP's Attachments tab and attach the signed AUP Renewal Form.
